Overview

District nurses are responsible for managing care within the community.

They do this in the patients own home, where they are housebound.   

District nurses:

  • support adults over the age of 18 years
  • assesses the healthcare needs of patients
  • monitoring the quality of care patients are getting
  • support patients to allow them to care for themselves
  • support family members to develop the confidence to care for their relatives if they're unable to care for themselves

Support available

District nurses work with the Children's Community Nursing Team or the appropriate health service to develop a plan tailored to individual needs. 

This includes young adults moving from children to adult health care. 


Who can access the service

People who are over 18 years old and housebound.


How to access the service

A referral will need to be made by a GP, hospital or local council.
