Overview

Hedley's Horizons is a programme combining Able 2 and Fir Trees, as part of the Percy Hedley Foundation.

The service offers practical and sensory activities that you can take part in throughout the week.

There is also a range of on-site facilities available. 

The service provides experiences that offer people:

  • help to become as independent as possible within the service and the community
  • a total communication approach:
    • use of British Signed Language
    • Signed Supported English
    • makaton
    • augmentative and alternative communication systems
  • support to make choices about everyday life and inform futures
  • help to live a life that service users would choose and get the most from
  • make sure they are treated with dignity and respect
  • have meaningful person-cantered individual outcomes
  • have a detailed person-cantered support plan as appropriate, working closely with them, their family social worker/care manager
  • help to try new things and develop your interests
  • a person-centred approach in everything we do
  • monitoring and development of health and welfare 
  • appropriate support based on assessed need
  • equal opportunities for all that respond to and recognise diversity

Support available

There is a nursing service that can provide support anyone with health needs, who need support from staff with specific training.

A therapy team is available without a referral throughout the year, as well as specialist treatment programmes when required.

A range of programmes are available including:

  • Activity programmes
  • Sensory programmes
  • Bespoke ASC programmes

Packages can be full or part time but do not include transport. This will need to be sourced by a care manager, social worker or family. 


Who can access the service

Anyone with special educational needs and disabilites, aged 16 years and older.


How to access the service

To access the service, you may be able to use your:

  • individual care plan and / or direct payments
  • personal care / health budget

If you do not have these, you can express your interest through your Social Worker and / or Care Manager.
