Overview
The Children and Young People Diabetes Service offers care to all children and young people aged 0 to 19 years old with diabetes.
They educate children, young people, and their families about their diabetes from diagnosis until the age of 19 years when they are transitioned to the young person's team.
Support available
If a child or young person has special educational needs or disabilities, the service will work closely with the child, young person, family and professionals that are involved in their care, aiming to empower the child or young person to manage their diabetes.
They will work alongside the key worker or educational establishment and adjust the format of educational tools used to suit that particular young person's needs.
This may be in verbal, picture, video or word format.
They will use British Sign Language interpreters if required.
Who can access the service
The service is open to all children and young people with diabetes.
They work with children and young people with an Education Health Care Plan (EHCP) in place so the child or young person is fully supported in all areas.
How to access the service
Children and young people are referred at diagnosis by the GP or emergency care.
