Overview
The service provides an annual eye examination for people aged 12 and over with diabetes.
Support available
The service supports patients in Sunderland and South Tyneside who have been diagnosed with diabetes.
Support for people with special educational needs and disabilities
The service works with charities to make sure eye screening is accessible to everyone, and any barriers are removed that stop access.
Some reasonable adjustments include:
- Easy read letters
- Double appointment slots for those who need more clinical time
- Fully flexible appointment booking so a patient can choose a day and time that suits them best
- Name contact clinician, to provide familiarity and reassurance each year
- A "Focus on Health" DVD to learn more about the screening process before attending an appointment
- A choice of 8 venues across Sunderland and South Tyneside
- Appointments up to 6pm for those who have carers or relatives who work during normal office hours and need a later appointment time
Who can access the service
Anyone aged 12 years and over with diabetes.
How to access the service
Referrals are from a GP.
After the referral, the patient is registered, and a record is created.
An open invitation letter is sent to invite the patient to book at a screening clinic venue of their choice.
